What We Do
ReferWell’s mission is to transform access to care and elevate the patient experience. We do that by using a curated provider database, an innovative Perfect Match℠ search and scheduling platform, proprietary member engagement and outreach from our care navigators and hands-on approach to program management that combined, eliminate administrative complexities for health plans and providers, seamlessly connecting patients with the care they need to ensure they get on and stay on their healthcare journey.
Appointment Scheduling & Follow Through
We take advantage of the referable moment℠ to schedule visits right on the call, more than doubling member follow through with recommended care. We send appointment reminders by email and text and engage members with post-visit satisfaction surveys.
Coordinated Care
We drive loop closure to 85% by making data sharing, care coordination and tracking part of the process.
Reporting
We report on behavior in real-time, giving insights needed to change behaviors, improve documentation and eliminate chart chasing.
Improved Access to Health Care & Services
ReferWell shortens the average wait time for a member to see a specialist by 25% with point-of-care scheduling and a broad network of community partners and specialists accepting Medicaid.
More Care Gaps Closed
ReferWell impacts 77% of Medicaid members we engage to close more care gaps. That includes gaps in care for colorectal and breast cancer screenings, colonoscopies, A1C and BP testing, and retinopathy screenings.
