Most Rural Providers Are Still Figuring Out

The Rural Health Transformation Program is one of the largest federal investments in rural healthcare in a generation. Understanding how to use it — and use it well — is the difference between incremental improvement and transformational change.

The RHT Program was created to help rural health providers manage the financial and operational challenges of a rapidly changing healthcare landscape. $50 billion spread across five fiscal years — $10 billion per year, from 2026 through 2030 — is available to states that submit qualifying applications.

Half the total is divided equally among approved states. The other half is allocated at the discretion of the CMS Administrator, weighted by a state’s rural population share, the density of rural health facilities, and the overall financial condition of its hospitals.

For organizations on the ground — hospitals, RHCs, FQHCs, behavioral health providers, and community clinics — the key insight is this: if your state participates, there will be federal dollars that can directly support work you are already doing. The program is designed to help you improve access, modernize care delivery, stabilize your workforce, and move toward sustainable payment models.
